Lincolnshire Partnership NHS Foundation Trust’s (LPFT) Adult Services Division provides all elements of acute care and secondary recovery teams.

The kind of help we can offer depends on your needs, taking into account your faith, cultural and social background.

It is very important that we work in partnership with you, your carer(s) and other family members. We will listen carefully to your views and take your wishes and feelings into account in all the work we do.

We will need to find out more about you to ensure you receive the best possible care, suited to your individual needs. This means we need to collect information, discuss this with you and agree what might be done and what services we can offer.

Referral/Access

NON EMERGENCY referrals to the Trust are triaged by the Primary Care Services, who have teams in each locality, who will refer on to the Recovery Teams, if appropriate.

EMERGENCY referrals will be triaged by the Crisis Resolution & Home Treatment Teams.
