Lincolnshire Partnership NHS Foundation Trust (LPFT) provides a range of services for people of all ages. These range from our child and family services, to substance misuse (drug and alcohol) services, and crisis services (rapid help in an emergency).
Wherever we can, our aim is to keep people out of hospital (in-patient services) - unless it is necessary – by supporting them in the community. This might be by providing care and treatment at the service user’s own home, or at another location within the community, such as a clinic or your GP‘s surgery.
The kind of help we can offer depends on people’s needs, taking into account their faith, cultural and social background. We think it is very important that we work in partnership with service users, their carer(s) and other family members, taking their wishes and feelings into account in all the work we do.
On these pages you can find out what each service does and who they are aimed at.
Do you need to access our services?
If you think you may need to use our services, please contact your GP. He or she will then refer you to us, if appropriate.
You may be seen in the first instance by a community psychiatric nurse from one of our primary care teams who will ask you a series of questions to determine what your needs are. This is known as triage and your appointment will be at your GP’s surgery or another place in your local community.
In an emergency
If it is an emergency, you will be referred to our Crisis Resolution and Home Treatment Teams.
